All forms must be turned in before an athlete can participate in a pole vaulting practice.  Click on forms below, print them and fill them out.  Please send the forms along with payment before the sessions begin.  I do not like to handle paperwork at the pole vault facility.  This gives me time to get everything organized before we begin the vault sessions.  Include your email address on the General Information Form in case I need to send out an emergency notice.

​Cancellation Policy

*Any sessions cancelled because of weather will not be made up by adding additional dates to the winter schedule.  Vaulters can make up at other scheduled practices throughout the winter session or will be given a free pass during practices scheduled during the Tune-Up sessions (Tune-Up Session are practices scheduled during outdoor track season.) 
*Go to altitudeheadquarters.com for cancellation updates before making the drive to our facility. Cancellations can be found on the top of the Vault Club Info page.  Sunday has three sessions scheduled.  It is possible one or two sessions will be cancelled and we still have the remaining sessions.
